Welcome to Sandalwood’s Long-Term Care Community, The Residences.  We understand that there are a lot of choices when it comes to long-term rehabilitation communities. The Rehabilitation Center at Sandalwood has served our community for over 50 years.  Community residents and families continually choose us to provide skilled nursing services for their loved ones. While many communities can sound the same on “paper,” you will not truly know if a community is right for you or a loved one until you visit. Tours of Sandalwood are encouraged so you can see firsthand how we are changing the face of long-term care.

When you determine that Sandalwood is the right choice, a Nurse Clinical Liaison will meet with you and your family to complete an assessment prior to admission. The goal of this meeting is to ensure we can meet your specific medical needs and your schedule preferences. Examples of information gathered during this meeting include:

• What time do you typically wake up and go to bed? This allows therapy and nursing to work around your schedule and preferences.
• What types of foods do you prefer? What are your dietary restrictions?
• What are your long-term care goals?

Resident Services

The Rehabilitation Center at Sandalwood employs a dedicated social worker to provide social and emotional support to our long-term rehabilitation residents. We help our residents access services within and outside of our care community, including:

• Arranging in-house health counseling and personal therapy sessions
• Helping with ancillary needs such as dental, hearing and vision
• Helping to organize palliative support when indicated
• Coordinating care conference meetings to discuss plan of care for residents and their advocates
