After the major addition of a new rehabilitation unit in 2009, and physical upgrades throughout the community, Sandalwood prides itself on extending exceptional hospitality to our short-term rehabilitation guests, long-term care residents and their families.

The Rehabilitation Center at Sandalwood is proud to be the oldest, privately owned and operated skilled nursing community in Jefferson County, Colorado. Over the past 54 years, Sandalwood has had the privilege of providing care for several generations of loved ones.
In 2011 Sandalwood partnered with the Planetree Foundation to work towards becoming a Planetree designated community. What does this mean to us? Becoming designated is a high honor in the world of healthcare- it means we have been recognized for providing and fostering a culture of placing our residents and guests first in the daily decisions involving their care and life in the community. Sandalwood has always believed that the resident/guest should be placed first and in partnering with Planetree we are gaining additional resources to further foster and enhance a resident-focused culture of care. What is Planetree? Since its founding in 1978, Planetree has continued to be a pioneer in personalizing, humanizing and demystifying the healthcare experience for patients and their families. Founded by a patient, the Planetree Model is committed to enhancing healthcare from the patient’s perspective. It empowers patients and families through information and education, and encourages “healing partnerships” with caregivers to support active participation.
